Release and Indemnification
I certify that I am physically fit and able to participate, or that I have discussed this with my physician and have received approval to participate. I understand that I should not participate if I have certain health conditions such as pregnancy, heart rhythm irregularities, and/or recent surgery. If my physician has placed limits on my activity level, I acknowledge that it is my responsibility to stay within these limitations and I agree to assume all responsibility for confining my activities to remain within those limits. I expressly assume all responsibility and risk including personal injury and possibly death, arising in any way from my participation in the FHN Get Fit INFINITE program. On behalf of myself, my heirs, executors and administrators, I agree not to sue and to release, indemnify and hold harmless, FHN, its affiliates, officers, directors, volunteers, and employees, and any sponsoring businesses and organizations and their agents and employees, from any and all liability, claims, demands and causes of action whatsoever, arising out of my participation. The results of any of my baseline and ongoing screenings will be accessible only to me and the FHN Get Fit INFINITE staff.
